Medical Billing is the Fastest Growing Opportunity in Health Care
The proliferation of medical knowledge following World War II brought about an explosion of diagnostic, and treatment procedures. As a result, there became a need to organize, and standardize all these developing technologies. Here is where the foundations of medical coding were born. Medical coding met these challenges, and allowed for a more uniform way of communicating health information under a common language. By January 1979, standardized definitions, and codes were adopted, and used by health care providers, and insurance companies. Since improvements and refinements of medical procedures are constantly being developed, codes must be added and updated to reflect these changes. Today, the number of medical and surgical procedures have become enormous, so too have the codes to describe them. These overwhelming numbers of codes and protocols have made outsourcing medical billing the standard.
Medical Billing and related occupations continue to be the fastest growing opportunities in health care. Insurance institutions and government agencies are investing huge resources to control claims' fraud, abuse, and establish some degree of reimbursement parity. As a result, more insurance companies, and health care facilities are looking to medical billing experts for help. These companies and practices are looking for experienced and educated individuals; the legal consequences of incorrect billing could be devastating. There is a movement in the industry to make medical billers responsible for inaccuracies, much they way accountants are for tax returns.
At present, there are no standards for educational requirements for medical billers and coders. However, more employers are looking for some formal training at an accredited career training institution. These schools range in training time from nine months to two years, anything less would not be considered adequate. There is a move for certification, and several organizations are sponsoring certification examinations in medical billing and coding. Medical billers and coders earn as much as $8 to $10 per hour in the beginning, and could potentially realize $30 to $40 per hours with experience and additional responsibilities.
Today, evolutions in the health care industry are happening at a very high pace. It is only with the assistance of sophisticated computer programs, and standardized coding procedures that medical coders are now able to describe and characterize the tremendous number and various kinds of medical and surgical procedures. These realities coupled with government regulations, and health insurance guidelines have contributed to the strong demand for experienced medical coders and billers.

One of the key initiatives in WHO is the development of lists of essential medical devices. A medical
device should be considered essential only when its use meets the basic needs of health services, has
been proven to be cost-effective, and is evidence-based (i.e. follows well defined specifications and is
validated through controlled clinical studies or widely accepted consensus by experts). Medical
devices should be selected with respect to their public health relevance based on need, efficiency,
safety and cost-effectiveness. A static list of medical devices is neither feasible nor useful.
Nevertheless, evidence shows that a template list of essential medical devices can assist countries to
plan and manage their needs [26,27].

Cardiac Emergencies, Are You Prepared?
Nurses and doctors are forced to know how to take care of a cardiac emergency, but what about you? I have two friends’ stories I would like to share with you briefly. Both of these two gals are medical professionals with very different outcomes. How do you want your friend or loved one to end up?
A few years back I had a wonderful colleague who was an emergency room nurse. She would soon turn 40 and was actually excited about it. She had a known problem of going into an unusual heart rhythm, but felt it was being cared for with medications. One night she felt a strange sensation in her chest, called out to her husband for help and dropped to the floor. Her husband was not trained in CPR and didn’t call 911 quickly enough. No AED or CPR for several minutes; precious time without oxygen to her brain. Against all odds, she survived and is talking and living today. She lives, however, with a brain injury in a long term care facility never to practice nursing again.
A year or two later I had a friend who was in her last year of med school. Her and her husband would soon be doctors and starting to work side by side in an internal medicine practice. One sunny day she chose to go for a run before starting her shift. She turned on her I-pod and started running. While out she collapsed on the street. A woman looking out the window called 911 and ran to her side. Knowing CPR she quickly started the steps of breaths and rescue breathing for the young doctor. Minutes later a police officer arrived with an AED in his trunk. Together the two hooked up the AED and shocked the young doctor. When the ambulance arrived they hooked up the doctor to the heart monitor to find she was no longer in a lethal heart rhythm and they could focus on getting her to the hospital for further treatment. Today the two doctors work side by side and play with their six month old son in their free time.
Learning CPR and ensuring that your business, church, school or emergency vehicles are equipped with a functioning AED are key to a good outcome. Neither of these two gals were older than 40. Both of them were well trained to care for other peoples emergencies, but only one had the emergent care that she needed. I encourage you to contact your local American Red Cross or American Heart Association to find a class near you and help to make sure your area is well equipped with an AED

Everything You Need To Understand About Pregnancy And Blood Disorder
Before we answer the question if you should be fearful or not or before we take and give advice from experts, let us first find out everything there is to know about ITP blood disorder. ITP blood disorder is a type of autoimmune disease that causes the body to attack or destroy its own platelet. We as humans are ready with what we call as anti bodies that combat viruses and microorganisms, preventing our bodies from getting infections. In the case of ITP, the very antibodies that we rely on to guard our systems are the ones attacking the platelets making their numbers lower than the normal amount necessary to keep us healthful.
What are platelets? Platelets are blood components. It is responsible for blood clot. They also help heal wounds faster. Without platelets our blood cell will not be able to clot, making us bleed excessively even on simple bruises and wounds.
Signs and symptoms
Symptoms and signs of ITP to lookout for are excessive bleeding, blood in stool and urine and regular gum and nose hemorrhage. People with ITP also have lesions and reds spots on their skin which are signs of bleeding under the surface of the skin. Both adults and children can have ITP but it is neither transmittable nor genetic, according to studies.
Should you be bothered?
Given all the facts about ITP, it is but average if you are pregnant to be concerned if you already an ITP patient given that it is a problem about blood clotting and hemorrhage. There may be concerns about delivering a healthy baby or not or delivery issues. These are all valid questions and concerns that we will try to address.
Although there are no studies that indicate women with ITP should not get pregnant, there will always be some risks involved for both the mother and the child. If you want to have a child and you know that you have ITP, consistent monitoring of the platelet count is very critical. At third trimester platelet count may drop drastically, so it is important to be observed regularly. That is why it cannot be stressed enough how crucial it is to have your platelets monitored and managed.
Can it distress the fetus or the new born? During pregnancy, the antibodies of the mother may cross the placenta. Upon entering the placenta, the antibodies may recognize that of the fetus’ platelets and can eventually damage them. Expecting women with ITP may therefore cause moderate to severe thrombocytopenia. But this situation is only impermanent. With proper diagnosis and care, thrombocytopenia in infants and kids do not have long term effects on their health. Thrombocytopenia on newborns can last up to eight weeks after birth. According to studies, women who have already given birth to children with thrombocytopenia can expect her other children to be born with the same disorder as well.
It is also worth knowing that children who develop ITP because of viral infection will heal itself as time passes by. They can recover even without medical intervention. But for your peace of mind it is still advisable to drop in on your doctor for proper management and caring tips to avoid bruising and wounds that could lead to blood loss

يلعب التدخين دوراً هاما بالإصابة فقد ثبت للعلماء إن التدخين يمنع بشكل قطعي إلتئام القرحة
الكحول والمسكنات مثل الأسبيرين ومركبات ال NSAID تقوم بتحفيز التقرح .
العامل الوراثي يلعب دوراً بالإصابة ولكن ليس دوما
زيادة نسبة إفراز الحامض المعَدي وإنزيم الببسين ، إن زيادة إفراز الحامض يؤثر في الأثني عشري أكثر مما يؤثر في المعدة .لقد وجد العلماء إن غالبية مرضى قرحة الأثني عشري يُفرز لديهم حامض الهيدروكلوريك أكثر من باقي المرضى أو من الأصحّاء ، لذا فإن قرحة الأثني عشري يمكن معالجتها بوساطة الأدوية المضادة للحموضة أو بقطع العصب المسؤول عن إفراز الحامض جراحياً .
قلة مقاومة الغشاء المبطن للمعدة لتأثير الحوامض
زيادةإفراز مادة الصفراء يؤدي الى تقليل مقاومة الغشاء المبطن للمعدة والأمعاء .
ووجد العلماء إن الإصابة بالقرحة الحادة ربما تحصل بعد إصابات الرأس ، الحروق ، الإلتهابات المعقدّة ، وأحياناً بعد التداخلات الجراحية أو الصدمات الجراحية .
الفحص السريرى
أهم ما يشكو منه المريض هو الألم وسيقوم الطبيب بالسؤال عن الاتى
.موضع الألم .
.علاقة الألم بتناول الطعام .
. الفترات الزمنية للألم .
ما يخص موضع الألم فغالباً ما يصف المريض ألمه على أنه شعور بعدم الراحة ،أو ثقل أو احساس بشيء غريب في هذه المنطقة أو ألم عام بصورة عامة أو حرقة مشيراً بإصبعه الى موضع الألم في منطقة تحت عظم القص ، وأحياناً فإن الألم يكون في الظهر ما بين لوحي الكتفين .
أما علاقة الألم بتناول الطعام ففي قرحة الأثني عشر غالباً ما يحدث بين فترات تناول الطعام ،ويسمى بألم الجوع ، هذا الألم يزول أو يقل بتناول شيء من الطعام ، أي بمعنى إن هذا الألم يزداد أو تبدأ نوبته حال كون المعدة لاتحتوي على طعام .
إن ألم قرحة الأثني عشر يوقظ المريض من نومه ليلاً ، و يزول أحياناً بتناول مضادات الحموضة .
أما قرحة المعدة فألمها يتزامن بعد ساعة أو أقل من تناول الطعام . إن نوبات الألم ترتبط إرتباطاً وثيقاً بنوعية الطعام الذي يتناوله المريض ، فالمريض نفسه يعرف أي طعام يؤذيه ، وكذلك فإن الألم يرتبط بالتدخين وبتناول الكحول وكذلك بالحالة النفسية للمريض .
بعض المرضى لايشعرون بألم وإنما يعانون إنتفاخا بعد الطعام ، بينما آخرون يحسون بدوار وأحياناً بفقدان للشهية مع حرقة في منطقة الفؤاد
الفحوصات الواجب عملها للمريض
فحص الأشعة التلويني( أي إعطاء المريض ، وحسب حالته الصحية العامة ، قدحاً من مادة الباريوم المشع )، يعد هذا الفحص من الفحوصات الشعاعية المهمة جداً في تشخيص القرحة وفي تشخيص مضاعفاتها ،مثل بعض التضيقات أو الإنسدادات ،حيث يأخذ الطبيب المختص بالأشعة العديد من الصور الشعاعية أثناء تناول المريض لمادة الباريوم الإشعاعية
الفحص بالمنظار يستطيع الطبيب من خلاله ان يرى القرحة أو مضاعفاتها بعينيه .كما يستطيع أن يأخذ عيّنة من بطانة الجزء الذي يفحصه .
و هناك أيضا فحوص مخبرية لتحديد و قياس حموضة المعدة و المعدل القاعدي لإفراز الحمض
أيضا الفحوص الجرثومية الخاصة بكشف وجود جرثومة الهيليكوباكتر بيلوري المسؤول الأكبر عن القرحة
العلاج
تنظيم الغذاء والابتعاد عن تناول الأطعمة أو الأشربة التي تؤذي المريض
الابتعاد عن الأسبرين ومشتقاته وعن التدخين والكحول ، مع التوجيه بتناول الحليب والخضروات الطازجة والأطعمة المطبوخة جيداً ، والابتعاد قدر المستطاع عن اللحوم الحمراء ذات الألياف.
تعالج القرحة بواسطة المضادات الحيوية والأدوية المضادة للحموضة وباستخدام مسكن الباراسيتامول عوضاً عن الأسبرين .المضاد الحيوى(الأموكسسلين) هو الأكثر تأثيراً على هذه البكتيريا .
أدوية تمنع إفراز الحامض في المعدة مثل أدوية التكاميت ، زانتاك ، رايسك ويجب ان تكون فترة العلاج بهذه الأدوية محددة
التدخل الجراحي . يلجأ الطبيب للتدخل الجراحى للاسباب الاتية
1. عدم حصول إلتآم للقرحة بعد إستعمال العلاج الدوائي .
2. حصول تحورات نسيجية في المعدة أو في الأثني عشري .
3 . معاودة القرحة بعد شفاء وقتي .
إن إجراء العملية الجراحية لإصلاح القرحة تبدو هي الخيار الأخير أمام الطبيب المعالج
الخطوات الوقائية
- الإقلاع عن التدخين ، وتنظيم وجبات الطعام وعدم تجاوز أي وجبة .
- تجنب المأكولات التي تهيج المعدة وتخفيف الإجهاد والإرهاق
